Pathogenesis of peritumoral edema in meningiomas. Part I: Metabolic examinations by MR proton spectroscopy: Teil I: Metabolische untersuchungen mit der MR-Spektroskopie

In contrary to intracerebral tumors, the genesis of peritumoral edema in meningiomas is not yet completely understood. The aim of this study was to detect metabolic changes and to discuss their potential role for induction of the peritumoral brain edema. For this purpose, MR proton spectroscopy (MRS) was performed in 9 patients with edema associated meningiomas. In 5/9 patients lactate (lac) was detectable in the peritumoral edema, each with extended edema of more than 50 cm3. In 3/9 cases with small or medium extent of brain edema (5.1 to 38 cm3) no lactate was found. Detection of macromolecule resonances in edema was possible in a case of recurrent meningioma using a self designed inversion recovery MRS-sequence. This study shows that ischemia is frequent in peritumoral edema of meningioma but most probably must be regarded as a secondary effect. In induction of edema, however, hydrodynamic processes possibly combined with migration of macromolecules from the tumor into the edema seem to play the most important role.
