A clinicopathological study of vascular tumors of the orbit, eyelid & conjunctiva

Purpose : Vascular tumors of the periocular region are among the most frequent encountered in these locations. They are histopathologically classified into cavernous (CVH) and capillary hemangioma (CPH). Imaging is widely used for the clinical diagnosis of vascular tumors of the orbit, which predominates in the intraconal location. This study aims to analyze the demographics, clinical presentations, and histopathological features of periocular hemangiomas located in the orbit, eyelid, and conjunctiva. Methods : A total of 51 cases of periocular hemangiomas were obtained between 2006 and 2023 from the MUHC-McGill University Ocular Pathology & Translational Research Laboratory. Demographic information including sex, age, lesion location, and laterality were collected and the clinical and histopathological diagnosis were reviewed. Results : From the 51 cases, 27 cases located in the eyelid, 22 in the orbit, and 2 in the conjunctiva. CVHs were predominant in the orbit (100%, n=22), while CPHs accounted for 70%(n=19/27) of the eyelid cases. Both CPH and CVH were equally prevalent in the conjunctiva. For the CVH of the orbit, the sex distribution was equal and the average patient age was 47 years old, For the 27 eyelid cases, 6 were CVH with an average age of 65 years old male sex and upper eyelid location predominance (67%, n=4). In addition, 19 were CPH with an average age of 54 years old, with female predominance (74%, n=14), and 63% (n=12) located in the upper eyelid. For the 2 conjunctival cases, 1 was CVH and the other CPH, both patients were male, with left eye location and clinical diagnosis matching histopathological diagnosis. Conclusions : To the best of our knowledge, this is the largest series of hemangiomas in these locations. All orbital hemangiomas represented CVH with no incidence of malignant transformation. CPH represented the majority of eyelid hemangiomas with no malignant transformation. In this series a surprise equal distribution of orbital CVH was found. There is a female predominance in eyelid CPH.
