Falls in PD result from dual dopaminergic–cholinergic loss.Cortical cholinergic activity provides attentional control of movement.Cholinergic loss "unmasks" fall risk resulting from striatal dopamine loss.A cortico-striatal circuitry model for falls is discussed.The circuitry model suggests therapeutic efficacy of α4β2* nAChR agonists.
