Assessment tools for chemotherapy-induced peripheral neuropathy (CIPN)

Background: Peripheral neuropathy still remains the significant side effect of chemoterapy, and is around 30% to 40% of the chemotherapy side effects. Several assessment tools have been developed to detect Chemotherapy-induced peripheral neuropathy (CIPN). Using assessment tools for CIPN should considered not only accuracy but also efficiency of practice. Objective: To systematically review the accuracy and practice regarding assesment tools for detecting symptoms and clinical grading of CIPN. Methods: Electronic search was conducted from 1980 to 2014 of databases including Medline, PubMed, CINAHL, and Cochraine. Eligible criteria were observational studies, published in English language, and the participants were adult cancer patients receiving chemotherapy. Two reviewers independently evaluated the title,and full text of the searched articles to determine whether they met inclusion criteria. References from selected articles are also searched. The quality of study was appraised using the Quality Assessment of Diagnostic Accuracy Studies (QUADAS) tool by two reviewers independently. Data extraction was conducted by the two reviewers. Results: Four hundred and eight studies were searched and 13 studies met eligible criteria. The assessment tools of CIPN from the 13 studies included physical examination- sensory testing, patient self-reported symptoms, and healthcare-provider assessment scale. Among the 13 studies, 6 studies reported reliability of the CIPN scale ranging from 0.38 to 0.96. Two studies reported validity of the CIPN ranging from 0.29 to 0.83. Discussion: The assessment tools for CIPN have different formats. The validity and reliabilty of the CIPN assessment tools has significant variation. Conclusion: The variation of formats of the assessment tools to detect CIPIN, shows that it is necessary to find an appropriate assessment tool for clinical practice.
