ACID-BASE BALANCE IN THE CEREBROSPINAL FLUID

Patients with systemic acid-base disorders are frequently confused or unconscious, but these clinical signs may not correlate directly with the severity of acid-base imbalance in the blood. In normal subjects, cerebrospinal fluid (CSF) pH, Pco 2 and bicarbonate values differ from those of the blood, and conditions which alter blood pH may alter CSF pH in the same direction, in the opposite direction, or not at all. The regulation of CSF acid-base balance and its relationship to neurological symptoms has thus attracted interest. The May issue of the Archives of Neurology 1 offers a review of the literature on CSF acid-base balance in man, and presents the arterial and CSF values for 48 patients with systemic acid-base disorders. Many of the patients were examined several times during their illnesses and treatment, in an attempt to correlate neurological states with changes in blood and CSF, pH, Pco 2 , and bicarbonate values.
