Levallorphan and meperidine in anesthesia; study of effects in supplementation of nitrous oxide-oxygenthiopental sodium anesthesia.

Meperidine hydrochloride is widely used to supplement nitrous oxide—oxygen—thiopental sodium anesthesia. It has been shown that its use reduces considerably the patient's thiopental requirements. If the respiratory depression caused by the meperidine is antagonized by the use of levallorphan, the amount of meperidine can be increased and thus a further saving of thiopental can be effected. This was shown by comparing the thiopental requirements of two series of patients undergoing surgery with nitrous oxide—oxygen—thiopental anesthesia. In one series of 462 patients, anesthesia was supplemented with meperidine alone, and in the second series of 501 patients, meperidine plus levallorphan was used. This study confirms the reports of others that levallorphan counteracts meperidine-induced respiratory depression and, by permitting the use of larger doses of meperidine, exerts a thiopental-saving effect.
