5: Co-occurrence of risky behaviors among adolescents with chronic conditions

Purpose To assess whether adolescents with chronic conditions (CC) differ from their healthy counterpart s in the number of co-occurring risky behaviors. Methods Data were drawn from the 2002 Swiss Multicenter Adolescent Survey on Health (SMASH02) data base, a survey including 7,548 [3,838 females ] in-school adolescents aged 16-20 years in Switzerland. Two groups were created: chronic conditions group (CC; N = 760, 50% females, mean age: 18.0 +/- 1.18 years, 36.3% students) included youth indicating that they suffered from a chronic disease or a disability needing ongoing medical care. All other youth were included in the healthy group (HG; N = 6,788, 51.7% females, mean age: 17.9 +/- 1.17 years, 33.2% students). Nine risky behaviors were analyzed: daily smoking, having been drunk in the previous month, having used cannabis in the previous month, having used other illegal drugs in the previous month, early sexual debut (before age 15 years), suicide attempt in the previous 12 months, eating disorder, violent acts in the previous 12 months, antisocial acts in the previous 12 months. There were no differences in age, gender, or academic track between groups. Groups were compared on each risky behavior and on the sum of them (categorized as 0, 1, 2, 3, and 4 or more) controlling for age, gender and academic track (student/apprentice). Results are given as adjusted odds ratio with 95% confidence interval. STATA 9 was used for the analyses. Results CC youth were significantly more likely to smoke (1.32 [1.13/1.54 ] ), to have used cannabis (1.32 [1.13/1.54 ] ) or illegal drugs (1.49 [1.15/1.92 ] ), to have had an early sexual debut (1.33 [1.03/1.72 ] ), to have an eating disorder (1.44 [1.07/1.94 ] ) or to have a ttempted suicide (2.24 [1.55/3.24 ] ), and to have performed violent (1.41 [1.16/1.72 ] ) or antisocial (1.48 [1.26/1.74 ] ) acts. No differences were found for being drunk (1.10 [0.93/1.29 ] ). When comparing the number of co-occurring risky behaviors, CC youth were significantly more likely to report three (1.53 [1.19/1.96 ] ) or four or more (1.98 [1.57/2.48 ] ) simultaneous behaviors. Conclusions Adolescents suffering from chronic conditions report higher rates of risky behaviors than their healthy peers. Moreover, they are also more likely report several risky behaviors at the same time. Health professionals dealing with this specific population of adolescents need to be aware of it and give them counseling and guidance.
