Alpha-fetoprotein and endodermal sinus tumor of the ovary

Serial alpha fetoprotein (AFP) determinations were made to follow a patient with endodermal sinus tumor and AFP levels were found to correlate with the size of palpable tumor. The reported case was a 13-year-old Black girl experiencing abdominal swelling polyuria and frequent headaches in April 1971. In May 1971 an exploratory laparotomy was performed and a 2500-gm ovarian mass was removed. 2 weeks later a complete hysterectomy was performed and no tumor was found. AFP was not detectable by Ouchterlony technique in June 1971. By March 1972 the mass recurred. Repeat AFP determination at that time using counterimmunoelectrophoresis was positive to 1:40 dilution of patient serum. AFP concentration was 20.6 mg% and chemotherapy was instituted. In July the mass increased slightly and AFP rose to 18.2 mg% and was easily detectable at a 1:10 dilution. By September 1972 the tumor had doubled in size and was fixed. Reexploration revealed a tumor which could not be resectioned and the patient died in October 1972. A tumor mass extract had an AFP concentration within the tumor of .4 mg/gm of tumor. Serum AFP at the time of tumor biopsy was .3 mg/cu. cm.
