Adjuvant treatment of patients with locally advanced (T3-4 or N+) rectal cancer after neoadjuvant chemoradiation and subsequent surgery is not well defined, and there is no consensus in clinical guidelines. Response to initial chemoradiation is an important prognostic factor, and a risk-adapted adjuvant treatment based on that is feasible and deserves future research.
